Abstract

The bolt of the SACH foot adaptor has failed by fatigue. The fatigue crack has initiated at the thread root and propagated progressively over more than 90% of the bolt cross section before culminating in an overload failure. No metallurgical or mechanical abnormalities were responsible for the fatigue crack initiation. Also, the bolt material was found to conform to the specification. Analysis shows that the most probable reason for the fatigue crack initiation is the assembly error in which the bolt was inserted into the socket adaptor to a depth of 10 mm instead of 20 mm. It appears that due to this, the bending stresses were more resulting in premature fatigue failure of the bolt. The general machining on the bolt was found unsatisfactory and hence it is recommended to improve the machining so that a better surface finish is achieved. A detailed analysis of the failure is presented in this report.
