Examination revealed that the splines of the gears in question have worn out by adhesive wear mechanism. This type of wear is generally observed when two metallic surfaces slide against each other under pressure. Study confirmed that neither metallurgical abnormalities nor excessive pressure on the splines was responsible for the failure. The failure is a progressive one and has taken place over a period of time. With the available information and evidences, it was not possible to identify the exact cause of failure
