Abstract

A failed pipeline of an aircraft was forwarded to the laboratory for analysis. The pipeline was found to have ruptured at the bent region adjacent to the weld located immediately after the pressure control valve. Examination revealed that the pipeline has failed by fatigue. There was multiple fatigue crack initiation along the circumferential as well as longitudinal direction of the pipeline. The fatigue crack initiation was not weld specific and mostly occurred in the parent material away from the weld. Analysis showed that the fatigue failure was related to inadequate flexural stiffness of the pipeline. No material defects and/or fabrication defects were responsible for the failure. A detailed analysis of the failure is presented and a few recommendations suggested.
